Our Approach
The Community Support service works in partnership with Cardiff Comunity Housing Association (CCHA) with funding from Welsh Assembly Government Supporting People Revenue Grant, to support 9 people accross CCHA housing stock.
Our skilled and committed staff work with people to support them to identify their goals and draw up a goal-orientated individual support plan which can include, for example, advice on budgeting, assistance with shopping, maintaining good relationships with neighbours and general counselling and advice.
Aims
The aim of the service is to encourage people who have long term mental health problems to develop the social and general living skills which will enable them to live successfully in the community.
Application and Referral
We will assess every application and give a written response stating why applicants have been rejected or accepted.
If successful with the application but there are no available properties, applicants will be placed on a vacancy list.
Acceptance
Once accepted onto the service the tenant will be allocated a Support Worker.
The Support Worker will explain the process of support, in particular, they will explain the tools they will use to document support the outcomes star, assessment of needs and support plans.
Support
We will meet with tenants regularly to discuss issues and to help them work towards greater independence.
Plans, assessments and outcomes stars will be reviewed on a regular basis with the tenant.
Withdrawal of support
We will withdraw support over time in a planned way, in co-operation with the tenant, funders, other agencies and sources of support where appropriate.
